Bramalea and Area Sub-Region Collaborative 

Sub-region collaboratives are groups made up of health care professionals, Health Service Providers, LHIN representatives and patients who collectively take an active role in leading health system planning and development in their localized geographic areas. 

The Bramalea and Area Sub-Region Collaborative will work toward creating innovative solutions to the unique challenges faced by their local population. Solutions will be driven through formal collaboration, active participation and the integration of services with the goal of improving the overall care experience and health outcomes for the patients, clients and their families in their assigned regions. 

The functions of the collaborative will include identification of their sub-region’s needs and priorities in partnership with the local key stakeholders and outlining a local action plan that aims to increase access to health care services and/or improve health related and performance outcomes.

Geographically, the Bramalea and Area sub-region encompasses all areas from Olde Base Road, South to Hwy 407 and, from Hwy 410/Hurontario Street, East to Gore Road/Mayfield Road/Hwy 50.
