Integrated Health Services Plan - IHSP 3

The Central West LHIN's third Integrated Health Services Plan (IHSP 3) outlines the Central West LHIN’s actions that aim to improve access to local health care services and to better integrate those services across the continuum of care, for the period April 1, 2013 to March 31, 2016.

The Central West LHIN did extensive community consultations with local residents and Health Service Providers (HSPs) to find out what they thought the LHIN should work on during this three-year period. Feedback was used to develop strategic directions that are guiding the Central West LHIN to improve health care services across local communities.

IHSP 3 aligns with Ontario’s Action Plan for Health Care, the Ministry of Health and Long Term Care's (MOHLTC) strategy that is focused on building a health system more responsive to residents, while delivering better value for money. It also aligns with pan-LHIN system imperatives, which focus the collective efforts of all 14 LHINs on navigating and facilitating the transition of residents across the health care system and between health care providers.

The following four strategic directions and accompanying initiatives, outlined in IHSP 3, are currently being used to guide Central West LHIN activities for the period April 1, 2013 to March 31, 2016:
